Adding a co- applicant to your account allows the same access and responsibility as the primary applicant.
Co - Application Form:
1. UT-UTS-01.03 (E) Co-Applicant Form PDF
- To add someone your account.
- A valid drivers license, state ID, or passport is required.
2. E-mail, Mail, or Fax the form(s) with a copy of your valid ID.
- Send an E-mail with the form name in the subject line.
- Fax co - application form to (505) 891-5204. Call Customer Service to confirm receipt of fax.
